In this section enter the information that appears in the top left corner of the check. This would typically include your first and last name, your street address (incl. any apartment number), city state & zip, and phone number. Nowadays, with all the new ways of communication, you may want to put in your cell phone number & email address. (But not your Facebook or Twitter IDs, please.) There is room for 5 lines, but there is no need to fill all of them. The only required field is the first line which must contain the account holder�s first and last name. If there are two primary account holders, both names should be listed on separate lines. All other personal information fields are optional, such as the address and phone number. However, many large retailers such as Walmart will not accept a check that does not have the address associated with the account preprinted on the checks, due to fraud and security concerns.
In this section enter the bank information as it appears on the welcome checks the bank gives you when opening an account. This includes the bank name, routing number (the first number that appears on the bottom row of the check), account number (the second number on the bottom row of the check), and the starting check number (any number you choose, but it should continue from the last number of the checks that you currently have). Deposit ticket routing number is only needed when your bank has a different routing number for deposit tickets (those white slips found in the back of the checkbook). Most banks use the same routing number, so if you aren�t sure, leave this field blank.
Here is where you choose the style of how the information appears on your checks. Font choices are: Arial � the standard sans serif font that is used on most web pages, Times New Roman � the standard serif font used on printed pages, or Old English � an elegant and classy script. Text above signature is an optional field that can be used to write any kind of message that will appear above the signature area, such as �Void in 90 days�. If you aren�t sure what this is, just leave it blank.
