Form 1096 is a one page summary or compilation sheet that shows the totals of the information returns that you are physically mailing to the IRS. All businesses must submit a 1096 form with their 1099s. You must use a different form 1096 for each type of 1099. The laser forms come pre-printed and 5 to a pack.
